You are eligible to enroll in JHU health plan coverage as long as you are a full-time, part-time, or limited-time faculty, staff or bargaining unit member.
In addition to yourself, you may also cover your eligible dependents. Dependent eligibility and documentation requirements are shown below.
Spouse
- Copy of legal marriage certificate (English translation) or Affidavit of Marriage/Domestic Partnership
Domestic partner
Biological child until end of the year child turns 26
- Copy of birth certificate, or
- Verification of birth from hospital (must contain parent’s name)
Step-child and child of employee’s domestic partner until end of the year child turns 26
- Copy of birth certificate or Affidavit of Marriage/Domestic Partnership showing your spouse/partner’s name, and
- Copy of the marriage certificate showing the employee and parent’s name
Adopted child until end of the year child turns 26
- Copy of proof of legal guardianship
Child for whom you are the legal guardian, including children placed for adoption until the end of the year child turns 26
- Copy of proof of legal guardianship
