Eligibility & Dependent Certification Requirements
You are eligible to enroll in coverage under the plans described on this web site as long as you are a full-time, part-time, or limited-time faculty, staff or bargaining unit member.
In addition to yourself, you may also cover your eligible dependents. Dependent eligibility and documentation requirements are shown below.

|Biological child until end of the year child turns 26||Copy of birth certificate
Verification of birth from hospital (must contain parent’s name)
|Step-child and child of employee’s domestic partner until end of the year child turns 26||Copy of birth certificate or Affidavit of Domestic Partnership showing your spouse/partner’s name; and a copy of the marriage certificate showing the employee and parent’s name|
|Adopted child until end of the year child turns 26||Copy of proof of legal guardianship|
|Child for whom you are the legal guardian, including children placed for adoption until end of the year child turns 26||Copy of proof of legal guardianship|
